- Fully Coordinated is ideal for individuals who want the choice and flexibility of selfmanagement but also benefit from the support of a Care Coordinator, who will engage services as directed by you!

ADDITIONAL FUNDING STREAMS
IIn addition to your core care funding, you may be eligible for extra support:
Assistive Technology and Home Modifications
Eligible clients can access up to $15,000 in lifetime assistive technology funding (e.g. mobility aids), with exceptions only if clinically reassessed and approved. Plus a separate $15,000 for home modifications (e.g. ramps, rails, structural changes).
Restorative Care Pathway
Up to $12,000 over 16 weeks for short-term, goal-focused recovery support.
End-of-Life Pathway
Up to $25,000 over 16 weeks for care focused on comfort and dignity in the final stage of life.

Client (2-Step) Sign Up Process
Once the steps above are completed, services listed on the care plan and budget can commence on the start date and the clients, or you can commence sending invoices to Trilogy Care for payment.
